Registration Information
We are sure that this will be a sell out event! Your registration includes:
Unlimited workshops, all weekend activities, Saturday night banquet, UWD 2010 t-shirt, and souvenir book.

April 1, 2010. Any registration form received after April 1st (4/2 - 15/2010) will require an additional
$10.00 for a total of $95. No registration forms will be accepted after April 15, 2010.

Please note the following:
No Refunds / Replacements ONLY

ALL guests must sign the registration form; no monies will be accepted without a signature.

Seating for Saturday's banquet will be assigned based upon receipt of your registration. Groups who wish to be seated
together must included ALL REGISTRATIONS in one envelope.
